What you need to know about Driving Records.
The Division of Motor Vehicles is required to keep personal information on driving records confidential pursuant to RSA 260:14. The information is only available to certain persons who need the information to conduct their businesses, such as law enforcement, insurance companies, government agencies, or the individual themselves. Persons requesting records are screened to see if they fall within one of the permissible user groups. Driving records, with or without personal information, may not be obtained without consent or appropriate permission.
The Division of Motor Vehicles administers and enforces motor vehicle statutes that have a direct effect on an individual's driving record, the status of license/operating and vehicle registration privileges. It is the central site for data gathered from all law enforcement agencies, County, District, Superior and Circuit courts, and out-of-state motor vehicle registries.

Driving Record
In addition to the historical records (suspension/revocations, restorations, convictions, and/or crash involvement) each record type contains the following information:
- Name
- Date of Birth
- Address – that shown reflects the most current address appearing on the files of the DMV
- New Hampshire identification number
- Suspension/Revocation status of the license/operating privilege
- Insurance filing/SR-22 requirement (required or not required)
- Completion date of Safe Driver Course (appears only if completion certificate is provided to DMV)
- Completion date of Attitude Program (appears only if completion certificate is provided to DMV)
- Name(s) the individual is also known as (AKA) and/or was formerly known as (FKA)
- Information related to the individual's driver license is limited to:
- Current Driver License issue date and expiration date
- Driver License Classification
- License Restrictions
- Demerit Points
- Miscellaneous information related to driver license classification
Please note: Social Security Numbers are NOT contained to a motor vehicle record.
Requesting/Releasing your own Driving Record
Requesting your own Driving Record
You may request your own Driving Record either online or by submitting the following information in an envelope and placing it in one of our Drop Box locations:
- Release of Motor Vehicle Records (DSMV 505)
- $15.00 fee (made payable to "State of NH-DMV")
Releasing Your Driving Record to Another Individual or Entity
You may release your Driving Record to another individual or entity by submitting the following information in an envelope and placing it in the Concord Drop Box ONLY:
- Release of Motor Vehicle Records (DSMV 505)
- You must complete Step 3 by filling out the appropriate information for the third party that will be receiving your record.
- Your signature in Step 4 must be notarized by a Notary Public/Justice of the Peace
- $15.00 fee (made payable to "State of NH-DMV")
Requesting/Obtaining someone else’s Driving Record
Requesting someone else’s Driving Record
You may request your own Driving Record either online, by making an appointment, or by submitting the following information in an envelope and placing it in one of our Drop Box locations:
- Application for Motor Vehicle Records (DSMV 505)
- $15.00 fee (made payable to "State of NH-DMV")
Obtaining Another Person's Driving Record
You may obtain another person's Driving Record by submitting the following information in an envelope and placing it in the Concord Drop Box ONLY:
- Application for Motor Vehicle Records (DSMV 505)
- The named individual in Step 3 will need to have their signature notarized. If the individual's signature has not been notarized, then Step 5 "Intended Use of Information" must be completed.
- $15.00 fee (made payable to "State of NH-DMV")
Obtaining Another Person's Driving Record - Second and Third Party Requests
If you are acting as an authorized agent of your employer or a company, a Certificate of Authority must accompany the completed Release of Motor Vehicle Records (DSMV 505) or one must be on file with the DMV. Please note that these requests may only be processed by utilizing our Drop Box at the Concord DMV office or you may mail the completed form, along with $15 payment to:
