What you need to know about applying for a Duplicate Driver License, Non Driver ID or Commercial Driver License
Lost or Stolen License
When you drive a motor vehicle in New Hampshire, you are required by law to have a driver license with you. If your license is lost, stolen or destroyed you must apply for a duplicate license.
Apply for a Duplicate License or Non Driver Identification
If you have lost or misplaced your Driver License or Non Driver Identification Card, you may either utilize our online service in which your permanent license or non-driver identification card will be mailed to your New Hampshire mailing address, or you may make an appointment.
If you wish to make an appointment, please bring the following:
- Completed and signed Application for Duplicate Driver License or Non Driver ID (DSMV 637)
- Appropriate Fee
Temporary Duplicate License by Mail
If you are temporarily away from New Hampshire and your license is lost or stolen, you may apply for a temporary duplicate license by mail. If your image is not on file at the DMV, you will be unable to get a duplicate license by mail. To apply, please send the following documents:
- A completed and signed application for a Duplicate Driver License or Non Driver ID Card
- A Temporarily Away form providing an out-of-state address.
- Appropriate Fee

The DMV will mail a temporary duplicate license which will be valid for six (6) months to your out-of-state address. Once you return to New Hampshire, you are required to submit a new Duplicate Driver License form through our Drop Box services, along with a note explaining that you have returned to the State and a photocopy of your temporary license, and a permanent license will be mailed to you.
Lost or Stolen License

- Commercial Driver License Application (DSMV 312)
- Copy of current Medical Card (unless you are exempt)
- Appropriate Fee
Temporary Duplicate License by Mail
If you are temporarily away from New Hampshire and your license is lost or stolen, you may apply for a temporary duplicate license by mail. If your image is not on file at the DMV, you will be unable to get a duplicate license by mail. To apply, please send the following documents:
